3. How to make a referral

Medic shaking handsReferrals are accepted both from individuals and from participating GP's in the Aberconwy area. During the programme, the referring medical practitioner maintains overall medical responsibility for each participant.

The 'Gateway to Serenity' programme is especially suitable for stress, generalised anxiety disorder, social anxiety, panic disorder and mixed anxiety and depressive disorder (where anxiety is the most pressing concern). See our brief referral guide (215k PDF) for more details.

You can download the GP referral form here (370k PDF). It can be completed online, then posted or faxed to Aberconwy Mind offices in Llandudno. Please 'phone (01492) 879907 for more information or fax the completed form to (01492) 864041. Alternatively, you can download a self-referral form here.

Assessment

HADS or GAD-7 & PHQ-9 are expected to be less than 15 on referral. Higher scores may be acceptable if social & occupational functioning is relatively unimpaired (GAF > 50). The GAD-7 (680k PDF), the GAF (290k PDF) & PHQ-9 (870k PDF) are available from our tests page.

The supported programme is currently only available in certain parts of North Wales, UK. Please don't send a referral if you're outside an area where this service is provided, as we probably won't be able to help!
